Kentucky IMPACT Plus Case Management

Case Management programs designed to serve children between the ages of 3 and 18. The children are diagnosed with Severe, Emotional and Behavioral Disorders (SED), and are at risk of being institutionalized. The basic goals of the programs are:

  • Maintain the child in the least restrictive environment for their needs, by utilizing community resources and the wrap-around process.
  • Facilitate Service Team meetings every 60 days with all professionals and family members in order to create a comprehensive Service Plan.
  • Provide the families with support and advocacy in various areas, including; mental health services, school, the legal system, and others.
